When you or the AI moves it's commander (or any other unit) over a n uncaptured City tile, onto/over a forest tile Right behind it with a hidden unit in it, the commander stops on the City tile and dies because he/she/it is at an illogical position.
The game is lost instantly! (or won if the AI or another Player does so, but the AI can obviously see "hidden" units in wood tiles made from multiple observations)

It can happen anytime during a battle, even in the last few turns. And it happened on the very first map we were playing, giving us a sudden "fun lost"-feeling and "bad quality"-impression.

This is a huge problem a few of my friends refuse to play the game now because it happened on a game we had been playing for over a month.

This is a feature, not a bug!

There are a few tiles that can be passed through by some units but not stopped into, like structures and bridges. You are intentionally taking a risk if you move (important) units into unscouted territory, where they are in danger of being ambushed on a critical tile. Even the codex warns about this. Thus, if your commander dies this way - too bad. Learn from your mistakes and start a new game. Blame yourself, not the game.

Seems to be the ambush mechanic indeed which only applies in "fog of war"-enabled games. This brings in another tactical aspect to take care of and also allows for some interesting tactics for baiting your opponent into this situation. Imo this is an intended feature.

I was thinking about your post now for 10 minutes but this can't be intentional.
            - The game is instantly lost to begin with (that's not a game feature, that's frustration)
            - There is no indication of the possibility that the game is instantly lost on that move
            - There is no logical explanation, foreseeable causality or a connection to the real world
            - There is no skill involved on either team
Clearly a bug. If at all, it's an exploit but nothing more.

Its the ambush mechanic which is explained in the codex for "fog of war"-enabled parties. It also applies for naval units if they get ambushed under a bridge (stated in the codex). It makes sense that any unit ambushed on a occupied tile (city, etc) suffers from a significant disadvantage. Moving through uncharted territory during war can always lead to successful ambushs from the opponent. History shows that an unexpected ambush at strategical positions can have deadly impact on the victim (unless they got captured). Since capturing isnt part of the game you'll die.
E.g. an unprepared unit leaves an unknown city and gets ambushed at the city exit because the opponent could hide better near the city exit. This ofc doesnt apply on open fields because you will notice your enemy. In forests you can hide behind trees. In the mountains you can most likely use the terrain for cover. So it indeed makes sense that units die.
Also preparing such an ambush is tactics. The opponent running into the ambush is the tactic being successful. Rushing with your commander into uncharted territory and passing "unknown" cities in a warzone is not a good idea in general. The fog of war is not only a visual, it introduces an additional mechanic to take care of (and to even use it yourself). Planning and executing successful ambushes definitly involves skill because you dont want your ambush to look too obvious or even to fail.
There are 2 options in this case: either you avoid the moves entirely that might bring you into the 1hit situation or you play without fog of war.

However i totally agree on the point that this should be listed in the codex as well and not only the naval units. However i wouldnt consider this a bug or exploit.

Just looked it up and it is indeed described in the bridge Codex, thanks. That's the weirdest mechanic if I've ever seen one. The unit should just reset to the last possible location.
At least commanders shouldn't be able to be "ambushed" because the game is instantly lost in that situation or there should be a visual indication (yellow coloring of the area). It's just too easily overlooked and leads to frustration.

We had a match where the AI placed one of its units in a wood on our side of the map unknowingly. A friend of mine tried to tacticaly retreat his commander and died - wow, much fun, very strategic, AW reloaded, IGN 10/10.
