I was setting up a new room for a colonist, and when they were summoned they just vomited blood? (I don't have mods on)

I am pretty sure that this is not part of the normal game. I can't find anything in the files regarding that.
Whatever mods you have installed, they're not completely removed from your game.

NPCs often vomit if they use too much the teleporter or other interactive items. Some NPCs are clumsy by nature and they tend to shock themselves and vomit more then others do.

Now why is it red I dont know. But some things come to mind. Do you have a monitor or GPU setting (or both) that changes colors and lighting upon change of hour of the day? aka Mood Lighting or Ambient Light modes. Some TVs monitors or GPU come with such things ...

My ex girlfriend had a monitor setting that changed colors by an hour to "help ease the eyes" but everything looked weird to me even to the point sometimes I could not even look at the screen or could not see what is even on it because everything blended into one color ... sometimes some locations in the game were too bright or colors were rendering wrong. (in Starbound so as other games too)

So might not be the game itself but the device or background software you use.

The only thing I use is HDR (which I cant turn off at all) and often dark mode (prefering rather dark greys) whenever possible so I dont blast my eyes with the whites.
